Partial duals pay their own cost-sharing, so Hospital Indemnity + Accident are a legit pitch — but a cash claim payout can count as income/assets and put their Medicaid at risk. Keep it safe:

Size it to the copays. Keep the benefit small and copay-shaped so a payout reimburses their actual cost-sharing rather than exceeding it.
Two tests, not one. Income in the month paid (only sheltered if it functions as reimbursement of the real copay) + resources after. MSP resource limit runs ~$9–10k (some states have no asset test) vs ~$2k on full Medicaid — so e.g. $350/day × 6 days = $2,100 on top of light savings stays well under.
Coach the client. Apply any payout to the medical bill in the month received; they may need to report the change to their caseworker.
